To facilitate a smooth referral process, please provide:
✓ Patient name, DOB, contact details
✓ Reason for referral (suspected ASD, ADHD, or both)
✓ Brief relevant medical and developmental history
✓ Current medications
✓ Previous psychological or psychiatric assessments
✓ NDIS participant number (if applicable)
